Medicaid Home Care Clients in Illinois: A Guide for Agencies
Illinois funds in-home care through several state programs serving different populations. Here’s the landscape for agencies planning growth.
The programs (HFS / DoA)
Illinois delivers in-home services through the Home Services Program (HSP), run by the Department of Human Services; the Community Care Program (CCP), run by the Department on Aging for older adults; and managed long-term services and supports under HFS (Healthcare and Family Services). Illinois has moved to increase the base in-home reimbursement rate to about $30.80/hour (effective January 1, 2026, subject to federal approval) to support a higher minimum wage for direct care workers. Confirm the current rate and program rules against the official source.

What this means for agencies
- Know the program. HSP (DHS), CCP (Aging), and MLTSS serve different populations with different coordination structures.
- Care coordinators and case managers authorize services — relationships there drive referrals.
- Confirm eligibility and program fit before investing in a family.
